## Self referral to an accident and emergency department for another opinion.

OBJECTIVE--To determine whether patients referring themselves to an accident and emergency department for another opinion after consulting their general practitioner present with serious illness, show any risk factors for being admitted, or are more likely to be patients of particular practitioners. DESIGN--Six month prospective survey. SETTING--District general hospital's accident and emergency department, receiving 42,000 new patients a year. PATIENTS--180 Patients identified as attending for another opinion having already consulted a general practitioner. INTERVENTIONS--Classified as admission, referral to specialist clinic, follow up in accident and emergency department, or referral back to general practitioner. END POINT--Admission, with an analysis of admitted patients. MEASUREMENTS AND MAIN RESULTS--General outcome, diagnostic category, age, time of attendance, time since seen by general practitioner, and name of general practitioner were recorded. Forty seven patients were admitted, 99 were discharged back to the general practitioner (62 without a letter), and two died. Patients were most likely to be admitted if they attended within 24 hours after seeing a general practitioner, were aged under 5, or presented with respiratory or gastrointestinal complaints. Paulo Freire wrote, in his Third Pedagogical Letter: “If education alone does not transform society, without it, society does not change either.”(FREIRE, 2000, p. 67). In this perspective, education cannot restrict itself to the problems of the classroom. In its necessary ethic-politic dimension, it must contribute for the solutions of problems so serious today, that they concern with the surviving of the very humanity and of the planet as a whole. It is at stake, therefore, of thinking the education in terms of maximum urgency, or of planetary “limit-situations”. The main goal of this article is to quest in as much in works of different authors as in innovator projects, elements for that one I name “pedagogy of the great urgencies”. In a recent letter [PRL 78 (1997) 4581] Kob and Barrat reported results of their molecular dynamics simulation of the non-equilibrium dynamics of a binary Lennard-Jones glass. They propose an aging scenario in this system that is close to a t/t_w scaling behavior. ## A Simple Converse Proof and a Unified Capacity Formula for Channels with Input Constraints

Given the single-letter capacity formula and the converse proof of a channel without constraints, we provide a simple approach to extend the results for the same channel but with constraints. The resulting capacity formula is the minimum of a Lagrange dual function. It gives an unified formula in the sense that it works regardless whether the problem is convex. If the problem is non-convex, we show that the capacity can be larger than the formula obtained by the naive approach of imposing constraints on the maximization in the capacity formula of the case without the constraints. The extension on the converse proof is simply by adding a term involving the Lagrange multiplier and the constraints. The rest of the proof does not need to be changed. We name the proof method the Lagrangian Converse Proof. In contrast, traditional approaches need to construct a better input distribution for convex problems or need to introduce a time sharing variable for non-convex problems. We illustrate the Lagrangian Converse Proof for three channels, the classic discrete time memoryless channel, the channel with non-causal channel-state information at the transmitter, the channel with limited channel-state feedback. ## Convective Overstability in radially stratified accretion disks under thermal relaxation

This letter expands the stability criterion for radially stratified, vertically {unstratified} accretion disks incorporating thermal relaxation. We find a linear amplification of epicyclic oscillations in these disks that depends on the effective cooling time, i.e. an overstability. The growth rates of the overstability vanish for both extreme cases, e.g. infinite cooling time and instantaneous cooling, i.e. the adiabatic and fully isothermal cases. However, for thermal relaxation times $\tau$ on the order of the orbital frequency, $\tau\Omega \sim 1$, modes grow at a rate proportional to the square of the Brunt-V\"ais\"al\"a frequency. The overstability is based on epicyclic motions, with the thermal relaxation causing gas to heat while radially displaced inwards, and cool while radially displaced outwards. This causes the gas to have a lower density when moving outwards compared to when it moves inwards, so it feels the outwards directed pressure force more strongly on that leg of the journey. We suggest the term Convective Overstability" for the phenomenon that has already been numerically studied in the non-linear regime in the context of amplifying vortices in disks, under the name Subcritical Baroclinic Instability". ## Rainbow metric from quantum gravity

In this letter, we describe a general mechanism for emergence of a rainbow metric from a quantum cosmological model. This idea is based on QFT on a quantum space-time. Under general assumptions, we discover that the quantum space-time on which the field propagates can be replaced by a classical space-time, whose metric depends explicitly on the energy of the field: as shown by an analysis of dispersion relations, quanta of different energy propagate on different metrics, similar to photons in a refractive material (hence the name "rainbow" used in the literature). In deriving this result, we do not consider any specific theory of quantum gravity: the qualitative behavior of high-energy particles on quantum space-time relies only on the assumption that the quantum space-time is described by a wave-function $\Psi_o$ in a Hilbert space $\mathcal{H}_G$.; Comment: 4 pages, 2 figures. Worldwide, we are experiencing an unprecedented, accelerated loss of biodiversity triggered by a bundle of anthropogenic threats such as habitat destruction, environmental pollution and climate change. Despite all efforts of the European biodiversity conservation policy, initiated 20 years ago by the Habitats Directive that provided the legal basis for establishing the Natura 2000 network, the goal to halt the decline of biodiversity in Europe by 2010 has been missed. Hochkirch et al. (2013, Conserv. Lett. 6: 462-467) identified four major shortcomings of the current implementation of the directive concerning prioritization of the annexes, conservation plans, survey systems and financial resources. However they did not account for the intended network character of the Natura 2000 sites, an aspect of highest relevance. This response letter deals with this shortcoming as it is the prerequisite, over any other strategies, ensuring a Natura 2020 network being worth its name.

In the ongoing discussion on thermalization in closed quantum many-body systems, the eigenstate thermalization hypothesis (ETH) has recently been proposed as a universal concept which attracted considerable attention. So far this concept is, as the name states, hypothetical. The majority of attempts to overcome this hypothetical character is based on exact diagonalization which implies for, e.g., spin systems a limitation to roughly 15 spins. In this Letter we present an approach which pushes this limit up to system sizes of roughly 35 spins, thereby going significantly beyond what is possible with exact diagonalization. ## Millihertz Oscillation Frequency Drift Predicts the Occurrence of Type I X-ray Bursts

Millihertz quasi-periodic oscillations reported in three neutron-star low mass X-ray binaries have been suggested to be a mode of marginally stable nuclear burning on the neutron star surface. In this Letter, we show that close to the transition between the island and the banana state, 4U~1636--53 shows mHz QPOs whose frequency systematically decreases with time until the oscillations disappear and a Type I X-ray burst occurs. There is a strong correlation between the QPO frequency $\nu$ and the occurrence of X-ray bursts: when $\nu\gtrsim9$ mHz no bursts occur, while $\nu\lesssim9$ mHz does allow the occurrence of bursts. The mHz QPO frequency constitutes the first identified observable that can be used to predict the occurrence of X-ray bursts. If a systematic frequency drift occurs, then a burst happens within a few kilo-seconds after $\nu$ drops below 9 mHz. Glass is a microscopically disordered, solid form of matter that results when a fluid is cooled or compressed in such a fashion that it does not crystallise. Almost all types of materials are capable of glass formation -- polymers, metal alloys, and molten salts, to name a few. Given such diversity, organising principles which systematise data concerning glass formation are invaluable. One such principle is the classification of glass formers according to their fragility\cite{fragility}. Fragility measures the rapidity with which a liquid's properties such as viscosity change as the glassy state is approached. Although the relationship between features of the energy landscape of a glass former, its configurational entropy and fragility have been analysed previously (e. g.,\cite{speedyfr}), an understanding of the origins of fragility in these features is far from being well established. Results for a model liquid, whose fragility depends on its bulk density, are presented in this letter. Analysis of the relationship between fragility and quantitative measures of the energy landscape (the complicated dependence of energy on configuration) reveal that the fragility depends on changes in the vibrational properties of individual energy basins...

Si tratta di esperienze che è necessario indagare attraverso la prospettiva privilegiata della costruzione della biblioteca di Petrarca e del suo specifico amor librorum: perché di fatto l’avvicinamento all’arte —nei casi petrarcheschi qui studiati— avviene invariabilmente in nome di un distacco dal valore materiale di essa e a favore esclusivo, invece, di una superiore substantia spirituale riconosciuta all’esecuzione artistica. This research study focuses upon Credit Cards and a proposed verification method to reduce the risk of fraudulent use. It is anticipated that the proliferation and growing use of credit cards will require the use of security concepts offering built-in point of use protection that is automatic and reliable. A suggested concept is the use of a combined electronic credit card and a memory device. The proposed verification technique combines a user electronic credit card and memory number along with a simple imprinter which would be operated by the person accepting the credit card. Credit cards have become a widely accepted instrument of credit in the United States, and are a prime mover in the trend toward a "cashless society". It is estimated that there are 300 million cards in use today; growing at a rate of about 20 percent per year. Their increased usage and acceptance portends considerable change in credit instruments of many segments of the business world. It is also anticipated that the problem of theft and unauthorized use will increase in proportion equal to or greater than the card growth. The goal of this graduate thesis was to create 26 small felt sculptures of animals, creatures, and characters. These characters were designed to work together as one large twenty-six unit piece of work or in single units or small gcoups as smaller pieces of work. The entire group works together because the name of each animal or character begins with a different letter of the alphabet. It was a goal to make the animals and characters appealing and amusing. Bright colors were used as were simple facial features so they would appear friendly. Familiar shapes and animals were chosen to remind the viewer of items they might have seen or played with during childhood.
