Monitoring Drug Use Among HIV/AIDS Patients in Brazil: Should we Combine Self-Report and Urinalysis?

Illicit drug use in HIV-infected patients can be linked to impairment of physical and mental health, low health-related quality of life, and suboptimal adherence to HIV treatment. This study aimed to evaluate the correlation of self-report illicit drug use, urinalysis for cocaine and cannabis metabolites, and severity of dependence among HIV-infected patients on antiretroviral therapy (ART) in a treatment center in Brazil. Four hundred and thirty-eight outpatients of an HIV referral center were interviewed and assessed for drug use (lifetime, last year and last month). Urinalysis was performed to detect the presence of cocaine and cannabis metabolites in urine samples. Overall agreement between self-report and urinalysis was almost 68% for cannabis and higher than 85% for cocaine. Positive urinalysis was significantly associated with more than once a week cannabis (p < .0001) and cocaine (p <.0001) use during the last-month. Severity of Dependence Scale (SDS) properly predicted positive cocaine urinalysis results (area under the curve [AUC] = .81, p = .0001). Frequency of cannabis and cocaine use, SDS score degree and positive urinalysis for both drugs were correlated. A prática de divulgar relatórios de sustentabilidade tem se tornando comum em organizações por todo o mundo; em especial no Brasil, há uma forte tendência de crescimento na adoção desta prática. O objetivo deste trabalho foi identificar porque as organizações reportam e os aspectos positivos e negativos no modelo de reporte de sustentabilidade proposto pela Global Reporting Initiative (GRI), na opinião dos funcionários apontados nos relatórios de sustentabilidade como responsáveis por eles; estas informações são importantes porque poucas pesquisas investigam os relatórios de sustentabilidade a partir deste ponto de vista e porque oferece à GRI pontos específicos a serem melhorados. A pesquisa foi realizada por meio da análise de conteúdo, sem a utilização de softwares específicos para o método, das respostas dos funcionários de 27 empresas às questões: Por que elaborar relatório de sustentabilidade? AIM: The aim of this report is to describe the restorative treatment of an 18-year-old patient diagnosed with autosomal recessive hypocalcified-hypoplastic amelogenesis imperfecta (AI). BACKGROUND: Esthetic and functional rehabilitations in AI cases are challenging and should consider individual aspects, such as age, socioeconomic status, AI type, and intraoral condition. REPORT: AI was diagnosed in an 18-year-old patient. SUMMARY: Considering the short length of crowns and roots, patient life expectancy, minimal invasiveness, amount of treatment time required, and lower costs, oral rehabilitation with overdentures provided an adequate functional and esthetic rehabilitation of the patient. CLINICAL SIGNIFICANCE: This report demonstrated overdentures to be a viable, relatively inexpensive, and non-invasive treatment choice of a patient with AI with concerns about treatment longevity, invasiveness, cost, esthetics, and long-term maintenance.

This report is a summary of the activities developed during 9 months of curricular training at the Directorate of Risk Management for Medicines as part of the master degree in Pharmaceutical Medicine. The internship has the purpose of complement the theoretical knowledge previously acquired and was focused on the two areas of the Directorate: Safety Monitoring and Risk Minimization. The activities of the Directorate include the collection and validation of Individual Case Safety Report received from healthcare professionals, patients and pharmaceutical companies. The activities of Risk Minimization include the implementation of safety variations, management of periodic safety update report and risk management plan, validation of the direct healthcare professional communication and the evaluation of educational materials. Solitary juvenile xanthogranuloma (JXG) in the spinal column is extremely rare and there has been no report of such a lesion involving C1 and C2 in English literature so far. Here, we report and characterize the first case of xanthogranuloma of the upper cervical spine. This case report draws attention to the fact that solitary xanthogranuloma should be considered among possible diagnoses of spinal tumor in children and young adults. An 18-year-old female patient presented to the hospital with intermittent pain in the right side of the neck. MRI studies revealed a huge soft tissue mass to the right side of the C1 and C2 vertebras, and osseous destruction can be found in the cervical spine CT scan. Complete surgical removal of the tumor and occipital–cervical instrumentation with autogenous bone graft were performed with no complications. The patient was free of pain immediately after the surgery with intact neurological functions. Follow-up MRI 6 and 12 months after the surgery showed no residue or recurrence of the tumor. Our report and the literature review indicate that isolated JXG does not show any predilections of localization inside the central nervous system. So a solitary xanthogranuloma should be considered among possible diagnoses of spinal tumor in susceptible patients. Objective. To report a case of primary hyperparathyroidism in a pregnant patient, report the obstetric and neonatal outcomes, and review the relevant literature. Results. A 29-year-old primigravida was successfully treated for PHP with minimally invasive resection of a parathyroid adenoma in the second trimester of pregnancy. A healthy baby girl was delivered at 37-week gestation with an unremarkable neonatal course. To the best of our knowledge, this is the second case report in the literature utilizing intraoperative PTH during a parathyroidectomy in a pregnant woman. Conclusions. Primary hyperparathyroidism is a rare life-threatening condition that can present during pregnancy. The diagnosis can be difficult to establish during pregnancy, given the nonspecific symptoms related to hypercalcemia. However, a better understanding of the condition, improved diagnostic studies, and well-organized multidisciplinary management decisions can significantly reduce the morbidity and mortality associated with the disease during pregnancy. This case report is presented to highlight the value of early diagnosis and appropriate management of PHP during pregnancy.

Introduction. Many diseases and conditions can contribute to elevated liver enzymes. Common causes include viral and autoimmune hepatitis, fatty liver, and bile duct diseases, but, in uncommon cases like liver involvement in endocrine disorders, liver failure is also seen. Adrenal insufficiency is the rarest endocrine disorder complicating the liver. In the previously reported cases of adrenal insufficiency, mild liver enzymes elevation was seen but we report a case with severe elevated liver enzymes and liver failure due to adrenal insufficiency. Based on our knowledge, this is the first report in this field. Case Report. A 39-year-old woman was referred to emergency ward due to drowsiness and severe fatigue. Her laboratory tests revealed prothrombin time: 21 sec, alanine aminotransferase (ALT): 2339 IU/L, aspartate aminotransferase (AST): 2002 IU/L, and ALP: 90 IU/L. No common cause of liver involvement was discovered, and eventually, with diagnosis of adrenal insufficiency and corticosteroid therapy, liver enzymes and function became normal. The tissue bud that ultimately becomes the thyroid gland arises initially as a midline diverticulum in the floor of the pharynx. When the median thyroid analage does not descend in normal fashion, a lingual thyroid can result. It is thought that any thyroid tissue found in the lateral aspect of the neck, including around the vascular structures of the neck, may represent metastatic deposits from well-differentiated thyroid carcinoma. This is a case report of a 35 year old male presenting to the out-patient department with a swelling in the lateral aspect of the neck. The MRI report showed an enlarged level 2 lymph node in the lateral aspect of right side of the neck for which surgery was done and the specimen sent for HPE. Histopathological report gave a surprising report of thyroid tissue with normal follicular cells. But literature says that thyroid tissue lateral to the normal thyroid tissue is essentially metastatic papillary carcinoma. Only very few previous case reports showing the existence of ectopic thyroid in the lateral aspect of the neck have been documented (1,2,3). This case has been reported for its rarity.

This is a report which includes preliminary data from the year 2005 and description of potential data sources for National Health Account (NHA) in Kosovo. This year was chosen because it was the only year where data on health expenditures from all existing data sources was available. Natalja Jedomskihh-Eigo (consultant) prepared the report after her visit to Kosovo, 15 - 21 April, 2007. The results outlined in this report were presented and discussed at one-day seminar with stakeholders on 26 June, 2007. The report is intended as a discussion document. It represents the first implementation step of NHA in Kosovo to support formation of the Kosovo NHA technical team to lead the future work on health accounts. This report provides a brief description of existing data, including a review of available sources of information, identification of data gaps and recommendations for improved data collection.

Objetivos: Analizar el perfil de los pacientes que salen del Hospital con Informe Continuidad de Cuidados identificando las indicaciones más prevalentes. Ver el número total de altas con Informe Continuidad de Cuidados estratificado por especialidades. Metodología: Estudio observacional descriptivo cualitativo y cuantitativo, analizando todos los informes realizados de septiembre a diciembre de 2006 en unidades Médico-Quirúrgicas. Metodología cualitativa y cuantitativa. Fuente de datos: Servidor Gestión pacientes/Gestión de cuidados e Informe de Continuidad de Cuidados. Resultados: Cuantitativos: Emitidos 234 informes sobre un total de 676 altas, supone un 35% de pacientes con Informe de Continuidad de Cuidados. El 59,4% corresponden al área médica y el 40,5% quirúrgica, siendo Urología (57%) la unidad con más informes. Cualitativos: El 57% son mujeres y el 78% mayores de 65 años. Dissertação de mestrado integrado em Psicologia; Experiências adversas na infância são um fator de risco para comportamentos delinquentes na idade adulta. A investigação tem questionado a metodologia de autorrelato e os estudos que comparam o autorrelato retrospetivo e a consulta de registos oficiais, destas experiências, têm apontado inconsistências, sugerindo a vitimação autorreportada como melhor preditor de problemas futuros. O objetivo deste estudo é averiguar qual o melhor preditor do comportamento delinquente, a vitimação autorrelatada ou a vitimação dos registos oficiais, numa amostra de 29 jovens adultos, identificados como maltratados pela Comissão de Proteção de Crianças e Jovens. Os resultados mostram que as experiências adversas autorrelatadas predizem o comportamento delinquente (β = 0.392, p ‹ .05) explicando 15.2% da variância. Os dados do registo oficial sobre vitimação não predizem comportamento delinquente. As inconsistências vão no sentido de existir mais informação sobre experiências adversas nos registos do que autorrelatadas, o que levanta questões de ordem metodológica. This report reviews Community Driven Development (CDD) projects in Lao People's Democratic Republic (PDR) to determine their effectiveness in channeling resources to communities for poverty reduction. The study examines three CDD projects in depth: the Poverty Reduction Fund, the Village Investment for the Poor (both supported by the World Bank), and the Government-financed Village Development Fund. Through close analysis of these projects and cursory analysis of other CDD projects, the report concludes that overall, the CDD approach in Lao PDR improves the well-being of communities in a cost-effective manner. The study, however, identifies three challenges that remain for CDD projects to be wholly successful. The first challenge is a call for harmonization of CDD mechanisms within the country. CDD approaches are not currently coordinated geographically, technically, or financially. Objective. We report a rare case of Ewing's sarcoma of the zygomatic arch presenting in a 69-year-old patient. Method. Case report and a review of the world literature on Ewing's sarcoma incidence and management. Results. Ewing's sarcoma is a malignant round cell tumour of neuroectodermal origin that typically presents in the pelvis and long bones of children and adolescent boys. This report is the first to document the presentation of ewing's sarcoma of the zygomatic arch in a 69-year-old lady. Our patient underwent surgical excision and radiotherapy and at 4-year followup has no signs of recurrence or metastasis. Conclusion. To our knowledge this is the first case report to document Ewing's sarcoma of this location in a 69-year-old patient. This case report highlights the importance of diagnostic investigations in Ewing's sarcoma and discusses the management issues that this rare presentation raises.

Methods: Time spent in moderate and vigorous PA (MVPA) was measured at two time points in 1713 healthy individuals from 9 European countries using individually-calibrated combined heart-rate and movement sensing. Participants also completed the Recent Physical Activity Questionnaire (RPAQ), the short form of the International Physical Activity Questionnaire (IPAQ) and the short European Prospective Investigation into Cancer and Nutrition Physical Activity Questionnaire (EPIC-PAQ). Individuals were categorized as active (e.g. reporting ?150 min of MVPA per week) or inactive, based on the information derived from the different measures. Sensitivity and specificity analyses and Kappa statistics were performed to evaluate the three PAQs ability to correctly categorize individuals as active or inactive. Supported by a PhD candidate, they developed a report writing protocol with report templates, using action research as methodology. Collaborative steps, as part of an action research inquiry were taken to compile a concept report writing protocol and report templates. The concept protocol and templates were implemented in public healthcare vocational rehabilitation services. Critical reflection and feedback on the trial applications took place to refine the protocol. The protocol consisted of a cover letter, background information, legal and ethical considerations for reports, a step by step report writing guide including handy tips, report writing templates, and a guiding checklist. It concluded with a request for feedback from users, an undertaking for annual revision and suggested readings and skills training. Selected experts reviewed and critically appraised the final version. Dissemination of the final report writing protocol and templates was done through public healthcare forums and the annual vocational rehabilitation orientation workshop for occupational therapists entering public healthcare.
